Place Based Health in Housing Program - Supportive Housing Model for Tenants in Social Housing

Delivering place based health and social services to assist sustainable tenancies for vulnerable populations. OneBridge is an authorised alternative supplier for Naloxone and an approved member of the Needle and Syringe Provider Program. Within transition or social housing spaces, we focus on immediate access to:

  • Primary health care services: nurses onsite
  • Immediate access to specialist nurse practitioners via telehealth Australia wide

Health Outreach Co-Responder & Place Based Support for Specialist Homeless Services, Domestic Violence Shelters & Migrant Support Services

Our nurses deliver outreach co-responder and onsite health and social care services to people experiencing homelessness, women and children living in DV shelters, as well as specialist migrant support groups. We are dedicated to work closely with community groups across Australia to raise funding to enable immediate access to healthcare, and take the time it takes to discuss and navigate complex health issues for individuals and families. 


Additionally, 20% of funding raised from our Corporate Flu Program specifically provides essential support and healthcare to Australians experiencing homelessness and poverty. Our frontline nurses provide outreach services to selected charities each year, providing chronic disease management, health education, wound care, prescriptions, referrals, and immediate telehealth access to nurse practitioners.


This commitment underscores our dedication to making a positive impact on the lives of all Australians, regardless of their circumstance, resulting in equitable accessible healthcare for every human.
